Functional Capacity Assessments (FCA)

A Functional Capacity Assessment (FCA) is a report which identifies a participant’s current level of functioning, as well as what gaps in support exist. With this information, suitable recommendations, relating to the participant’s daily living challenges, are then also documented.

Examples of recommendations may be increased funding for support workers, allied health professional strategies or Assistive Technology (AT).

Information to determine functioning is gathered through interviewing the client, as well as administering of standardised assessments such as the World Health Organisation Disability Assessment Schedule (WHODAS) and Adolescent/Adult Sensory Profile.

Siobhan at Me and My OT also assists participants with progress reports which is a shortened FCA document required for a plan review.

Siobhan is experienced in writing FCA’s and can guarantee her work is written with exceptional skill, is thorough, and is completed with a fast turnaround. Having an OT who is highly skilled and experienced with FCA’s can be the difference between NDIS approval of recommendations or not.

housing reports SDA/SIL/ILO

Housing assessments work towards enabling participants access to suitable accommodation so that they may live as safely and as independently as possible.

There are many different housing pathways such as Specialist Disability Accommodation (SDA), Supported Independent Living (SIL) and Individualised Living Options (ILO), and Siobhan can help you decide which one may be best for your situation and needs.

A housing assessment is a collaborative process between OT and the participant, and involves collection of information about how the person is functioning within their current main home environment. Following discovery of needs, OT then develops a report with recommendations which is submitted to NDIS.

Siobhan is experienced in support participants in this area and can guarantee her housing reports are extremely high quality, thorough, and provided with a fast turnaround. Having an OT with experience and exceptional writing skill can dramatically increase your chance of having a submission approved by NDIS.
