Functional Capacity Assessments (FCA)
A functional capacity assessment (FCA) is a report which is developed to determine a participant’s current function, strengths and weakness so that recommendations can be made so that the correct amount of support is funded in your NDIS plan.
The report is created in partnership with the participant and involves completing assessments and information collection sessions to ensure the most accurate details are included.
Me and My OT also assist participants with progress reports which is a summary document required for a plan review.

housing reports SDA/SIL/ILO
A housing assessment is the development of a written report following assessment of the participants environment to ensure they are functioning adequately in their main living setting.
Housing assessments work towards enabling participants access to accommodation so that they may live as safely and as independently as can be.
There are many different housing pathways such as Specialist Disability Accommodation (SDA), Supported Independent Living (SIL) and Individualised Living Options (ILO) and OT can help you decide which one may be best for your situation and needs.

psychosocial
Me and My OT provide psychosocial services which aim to support participants to live meaningfully and at peace despite strong emotions and behaviours disrupting their functioning.
OT will work towards support improved quality of life through thought management, education about your condition, improved communication and social engagement as well as implementation of routines. OT will also complete assessments to determine if sensory or cognitive difficulties also require the necessary intervention.

daily living skills
OT’s are brilliantly placed to support improved daily living skills through their understanding of strategies and practical application of ideas.
Me and My OT can support participants to develop skills in areas of productivity, domestic and meal preparation, social and community.
Examples of daily living strategies include:
Time Management and organisation
Routine Building and schedules
Emotional skill development
Communication
Cognitive retraining

home modifications
Me and My OT can support home modifications following information collection and assessment relating to functional challenges or safety concerns around the participant’s home.
Following this, OT will write a report recommending custom changes to the home with improved accessibility in mind. For example, having a ramp built to replace steps so you are safer entering your home. This report is then submitted to NDIS for approval.
Examples of home modifications include:
Grab rails
Access i.e.. Ramp or platform step
Wet areas i.e.. Shower or toilet
Modified bench height

assistive technology
Me and My OT support assistive technology and equipment through first conducting information collection relating to the participants current functioning and goals.
Then a thorough functional assessment of the participants use of the recommended equipment, in the primary setting which it will be used, is completed. Following this, a letter of support is compiled and submitted to NDIS.
